
General: It is the policy of the Orthopaedic Specialists of the Carolinas to protect confidential information including credit card data from unauthorized access, use, disclosure, destruction, modification, or loss, whether accidental or intentional.
Applicability: This policy applies to printed information. Compliance with this policy shall be mandatory for each employee and shall be considered a condition of each employee's continued relationship with (Merchant Name).
Access to and Use of Confidential Information: Access to confidential information shall be based on the sensitivity of the information and the employee's need to know in connection with the employee's performance of his or her business responsibilities. Confidential information may be used only for business purposes and may not be used by any user for any personal purpose.
If you have insurance and we are contracted with your insurance plan, you will be responsible for co-payments and deductibles at the time services are rendered. We will however, file your health insurance as a courtesy to you. Please remember payment for physician fees is the patient's responsibility regardless of insurance coverage.
A late payment fee will be added monthly to accounts 90 days old. We accept cash, personal checks, money orders, Visa, MasterCard and Discover. If you have any questions regarding our participation with your insurance, you may contact one of our Patient Account Representatives at (336) 768-1270 prior to your appointment with any questions regarding our credit policy.
For your protection, prescription changes are issued only during office hours so that patient records may be evaluated. If you need a refill on your prescription, please call your pharmacy and not our office with those requests. If necessary, your pharmacist will call our office for authorization.
